This character profile template comes courtesy of Embri. I thought it might be useful to us here at TOCAF:
Name:
Well, we all have to call your character something... you may have more than one name for your persona, or things like titles, nicknames or true names can also go here.
Age:
Timeless? Newly born/created? You might want to include apparent age and/or actual age, if they differ.
Gender:
Girl? Boy? Neither? Both? Something exclusive to your species? It's all good.
Species:
That is, if you have one. If your character belongs to a species of your own creation, you may wish to include a detailed description under the next heading.
Description and Physiology:
Probably most relevant for the unusual or fantastic creatures. If your character can't be easily defined by a species, If you've got an unusual diet, habitat, metabolism or other such feature, this is a good heading to fill out.
Physical Abilities:
Can you fly? Swim? Dig? Run really fast? Shapechange? Jump tall buildings in a single bound? Received special training from some secret Military Agency? Abilities that are based on your character's physiology belong here, weather innate or learned.
Supernatural Abilities:
Magical powers? Spiritual prayers? Outer planar presence? Stuff that's not easily explained with basis on physiology should go here as a special power.
Signature Moves/Attacks:
If you've got a favorite tactic, special technique or similar action, this might be a good header to include for your profile.
Weaponry:
Guns? Swords? Teeth and nails? If your character uses something to fight with, this is where you can list it. Any special attributes of your weapon(s) might be good to include, as well.
Equipment:
Maybe you've got your own spaceship, or a portable wrist computer. Tech or items that aren't really weapons should go here. Try to stick to listing only things that are important to your character - you don't need to give an entire rundown of their wardrobe or personal hygiene kit!
Home:
Where your character lives, when they're not off on adventures. Simple enough.
Origin:
Everyone comes from somewhere... home planet, clan, tribe, city, world, universe... whatever works for you.
History:
A short (or not so short) summary of the pivotal events in your character's life.
Alignment:
The way you act, in general. Will you join the Dark side, or do you prefer to remain neutral? Completely random, chaotic, or logical and lawful?
Personality:
Happy? Sullen? Dark and brooding? In general, what mood is your character in most often. You could also include their ethos or code of conduct, if any. Anything in particular they like or dislike?
Known Weaknesses:
While some people will certainly not wish to include this header, I chose to do so because I think it helps present a well rounded profile. A good RP'er should embrace their weaknesses as an opportunity for good storytelling.
Allies:
Pet dragons? Familiars? Tribesmen from your old clan? These would usually be NPC's you call on or use in RPing, rather than other characters, though in some situations that may apply as well. If you have a VERY detailed Ally, you may wish to give them their own profile post.
